Unable to Apply Theme to Form

Do you mean you are unable to apply a theme to your form? I say that because the Templates which is cloning forms is different. Or have I misunderstood you? 

I checked on this and I can confirm you have 4 forms on your account. 

What theme are you trying to apply to your form? And for which form are you trying to apply it?

I tested applying form themes on my test account and it is working. 

Please clear your browser cache then try this again to see if that helps.

There seemed to be an issue with program.  I logged out and logged back in and it worked.  It was very weird.  But it is good now.  Thank you!!!!
