Form does not display correctly

My enagic application form code https://secure.jotform.us/form/51721051978154 does not bring up that form.

I send out an email with this same link so clients can fill out an appliccation,  I was working on the form am i must have hit something that made half of the form disappear.  i dont want to send out a different link to a huge database for obvious reasons.  i need the enagic form with the same link back.  when i click on stadard link it only brings up half the form.  help asap.

When i was adding and tweeking applic i think i hit something because the short version comes up when i clik on manditory link yet when i view it in my forms it iis the right form and the way it is suppose to be.

please help me asap  it is so important and i am crippled

If you have made an accidental edit to your form, please try using our Form-Revision-History feature to revert back to a known working version / revision of your form.

I would advise you to revert to the version before the change that was undesirable, and try re-adding what you were trying to add to your form.
