I am attempting to revert back to the classic or simple theme on my form, however everytime I save under designs or themes this does not work for the simple theme. I would like to use the Default Theme but when I do, everything comes up as Gray. How Do I fix this?

It seems that form is currently affected by applied custom theme. We do not have a direct 'remove theme' option at this time, so you might consider restoring your form with a Revision History tool (to remove previously applied theme).

It seems as though all of my revisions have the current theme. I have a few different forms on my account but this one is the only one where it has switched to the grey scale background and I cannot revert back.
I would like it the theme to be similar to what I have for the other two forms:

This issue has been reported to our developers by our colleague Mike. Until such time that we hear back from our developers on this, I can't see a way to undo the (Default) theme.
I do see that you are using the Default theme under Themes, and that you are using XXL Form theme under Preferences. At this time, I can only recommend that you try recreating your form again with the theme that you want to use.

Is there a way to duplicate this form without duplicating the theme otherwise we will have to start from scratch.
