Why has my form been removed from my website?

My web page www.antelopeexpress.com is supposed to have our form that we built on the site. Why am i paying money if this feature isn't working? I'm losing clients, and I need this resolved. I've already spoken with my web page person and everything is fine on their end. I need this fixed ASAP. I looked over the reservation form and the button on the bottom that says submit isn't working. Could that be it? Please get back to me right away.
