The forms are all automatically reset at the start of each month, I find it odd that you are the only person reporting this issue.
If it is something peculiar to your MadpenguinDesignsforms account, have you considered opening a 2nd account and cloning your form in to it?

I have checked your account and your current submissions (91) happens starting from November 1 to November 30, so there's no problem with your submission quota. Anyway, as what my colleague has stated above that submissions counter will reset every first day of the month (e.g. December 1, 2011).

Thank you for your hellp! Yes it did reset as excpeted. I just panicked a bit as it looked like it was counting sumbissions from back to September.... The form works awesome and is perfect for the needs of my clients. So THANK YOU so much for this service and for the amazing quick responses when I have a questions!!! I had a problem several moonths ago that it did not reset as it should have and you guys fixed that right away as well! Very happy with this site!!
I am interestied in the cloning the form like fxr mentioned? Could you tell me how to do this and can I actually set up a different account for my client to access with this cloned form? That would save me having to completely rebuild the form and give them their own control so I don't have to babysit it for them...

I am sure you could set up accounts for your clients and clone forms into their account, but NOTE: any changes you make to the form in your own account wont be automatically updated in their account.
