I set up two forms. Both get a "found not not, form is disabled" notice.
When I check the form properties the forms are enabled.
What have I done wrong and how can I fix this issue?

Your account and forms were auto suspended by the Phishing filter due to suspected phishing form that ask for Credit Card account details.
To be able to reactivate the account, you need to remove the suspected form http://www.jotform.us/form/51826268526159 under your account.
