    Hi,

    Yes this is very much possible in JotForm.

    You can use the same labels/question name for fields in the form, and hide them from viewers view. When data is downloaded to Excel, it will have column headers for each hidden labels/question similarly.

    You can hide the label/field names using custom injected CSS code.

    Follow the user guide: http://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes

    Kindly let us know if you need further assistance on this.

    Thanks.

    Hi - i'm not sure i understand this. How would i add a label to each of my questions in this form - http://form.jotformpro.com/form/51748363294968 ? For instance, for questions 1-5 I want to put a hidden label of A. Can you show me step by step?

    You can inject this CSS codes

    #label_5,#label_6,#label_7,#label_8,#label_9{

       visibility: hidden;

    }

    see the animated image below for guidance.

    Let us know if you are still not able to do it.

    Thanks.

    You can use the TEXT field to add the question text like this.

    I use CSS codes to move the question a bit closer to the ratings field

    #id_5 {

    margin-top : -50px;

    }#id_6 {

    margin-top : -50px;

    }#id_7 {

    margin-top : -50px;

    }#id_8 {

    margin-top : -50px;

    }#id_9 {

    margin-top : -50px;

    }

    here is my test form http://form.jotformpro.com/form/51827559161966?

    You can clone the form if you like and check it on your Form Builder.

    follow this guide to clone -How-to-Clone-an-Existing-Form-from-a-URL

    Hope this help. Let us know if there is more we can assist you with.

    Thanks.

    Hi - that really doesn't work - in the excel document it doesn't give me a row that i can work with that i can create sub categories of questions or group questions based on another label

    It seems like your question is still related to the original thread you have here: http://www.jotform.com/answers/600660.

    I'm not sure if I understand you correctly. But, each question field, in this case the rating field will have it's own column, in one cell. And one row for each submission.

    Have you tried using the Matrix field instead? It groups the answers in one question.

    I hope that somehow it gives you an idea for a workaround.

    Thanks.
