JotForm is a free online form builder which helps you create online forms without writing a single line of code. No sign-up required.
We believe that if one user has a question, there could be more users who may have the same question. This is why many of our support forum threads are public and available to be searched and viewed. If you’d like help immediately, feel free to search for a similar question, or submit your question or concern.
It seems like the demo form is outdated. But you can still follow the steps mentioned in the guide, it should still work. It should work on a Flash website.
If there's an HTML editor in your website, you could try our embed codes in the "Embed Form" options.
See if that works. Let us know if you are still having problems embedding it.
We'll wait for your response.

The guide mentioned here "Add a Form to Flash", is a step by step on how to embed your JotForm to a Flash based website created using the software ActionScript 3 versions of Adobe Flash.
If you are embedding your JotForm to your Flash based website, please let us know the website editor that you are using. If you are adding the form to a flash document, which is a separate file and not a website, I'm not sure if that's possible.
