Form Disabled/Suspected of Phishing?

    When I try to link someone to a form (i.e. the photo release form), the link brings them to a message that says "Form not found". Also, when I try to embed an email, the platform tells me I have reached my limits. I have not yet sent any forms, so how could I have reached my limits? When I initially tried to login today, there was a message saying this accoutn was deactivated due to suspected phishing. How can I rectify this??

    Please let me know if I did anything wrong, and if this can be fixed.

    I see that your JotForm titled "OVC, INC. Automatic Payment by Credit Card" is collecting credit card information on plain text fields which is not allowed through JotForm. Such forms are considered as phishing forms. Please refer to our terms of usage for more information on phishing. In order to receive any payments through JotForm, you'll need to use one of the payment integration tools that are secure and comply with PCI Data security standards.

    I've now activated your account and we request you to delete the guilty fields from the form to agree with our terms of usage. The account shall be suspended again by our anti-phishing system if the fields are not deleted from the form.

    Ok, I'll delete it.  I'm so sorry!  I had a difficult time creaing that one because I didn't now what items to use.  That helps a lot.  Thanks!
