It will be easier to identify the issue if the client provides the particular problem, Is it possible to obtain this specific information from the client.
So far the logic of the form is correct and it doesn't show any errors or image selection difficulties.
Also I received the email notification after filling it.

Just to add my 2 cents worth, one issue I noticed was that even though the form is in Turkish, the required field error message is in English:
