    Hi,

    You can import your fillable PDF to JotForm by using this Import Tool: http://www.jotform.com/adobe-formscentral-alternative/. Click the "Import Forms" button and choose the 3rd option:

    After you imported it, it might take a couple of days before it shows up in your account. When the PDF form is in your JotForm account, you need to re-download it for you to use it properly.

    You will then have an option to auto collect the form submissions using that newly downloaded PDF form.

    Please do take note that the fillable PDF forms works best when opened in Adobe Acrobat Reader or Foxit.

    Please also do take note that imported PDF forms cannot be edited in our form builder. I would recommend that you recreate the form using our form builder so that you'll have an online or web version of it.
