Forms being disabled

I am getting a message indicating this form is disabled but it is not when I check the settings. I do not know what has happened to this form?

I have checked the account of the owner of the form: http://www.jotform.com/43435161949965 that was disabled.

The account was suspended due to violating our Terms of Use regarding Phishing. 

We found out that most of the forms created on that account collects sensitive information such as Social Security Number(SSN) and Credit Card Information. 

Please be advised that we have an Anti-Phishing System that checks every form and marks the forms that are found suspicious of Phishing Activities then automatically suspends the account. 

The account can only be reinstated or unsuspended if the forms are deleted.
