I need your phone number... Why is there no way to contact you guys except an email? Thats absurd.
I will soon have accumulated many Medical History Forms (captured by your system) and as I need to locate older forms, by patient name, Im having more and more issues having to go through each and every form one by one...
Is there not a search option to help me find a specific patient that filled out a form 6 mo ago, or do I have to keep going one by one searching for the information I need.
I need to resolve this immediately. I can't have my patients filling out forms over and over again b/c "we can't find them".

Referring to your question do you mean searching submissions within a particular form or searching for a specific form in your My Forms page.
To search for forms you can use the search box on the top right and you can also sort you forms based on a number of parameters
If it is searching for submissions in a particular form, you can filter by timeframe or you can use the grid search at the bottom of the submissions page and type the patient name
