Can I include a "feedback" text field at the end of my form?

I want to include an option for users to submit feedback that will be sent to a different email address.
Can this be included on the Thank You screen?

I wanted to add a place where once the form has been submitted the form will be sent to one email address, the user can give their feedback and that feedback will go to a different email address.

I created an image of what I'd like to do.

What you could do, is create a second form that looks exactly like the thank you page you described.  Then have the first form redirect to that form instead of a Thank You page.  Something like this:

Then set the form URL to be your custom thank you page URL:

You can setup the notification for the feedback form to be different from the first form.  In order to change your recipient email addresses please refer to our user guide on How to send notifications to multiple email addresses

The process of changing a single recipient email address on your form notifications is just the same on that user guide.
