Integration with Mindbodyonline

Is there a way to connect a form with https://clients.mindbodyonline.com/?

I need to be able to create an online waiver form that is connect to my mindbodyonline crm program,

Unfortunately, we do not have a payment integration for Mindbodyonline at the moment.  There have been a few requests for this feature, though each one has been given a low priority so it does not look like it will be added any time soon. 

Zapier is currently having a Vote to integrate JotForm and MindBody:

But they do not yet have an integration setup either.  They may be your best bet though.
