How to import my existing clinic forms into Jot so that I can create a Jot form?

    Hi,

    It depends on what you are importing. May I know where your forms are? Are they PDF forms? Paper forms? Or an online form from another website?

    Thank you for your response.

    Currently they are forms which my patients fill out when they arrive at my office. I would like them to be able to fill these forms out online before they arrive for their first appointment visit. Do I need to manually recreate these forms in JOT? Please advise.

    Hi Roberta,

    The form importer support forms created only using adobe forms central, If you did not create the forms using the adobe tool you will need to create them manually using the form builder tool.

    Once you have created the form you can generate a fill-able PDF that you can send to your clients
