How can I build a form that has a spreadsheet with an upload file column??

I need to set up a form that is like a spreadsheet and customers can upload files.

The form should be set up with the following columns

Company Name          Category           Name             Description            Contact Information       Company Logo --->

Enter                           Make Selection         Enter Info        Enter Info            Enter Info                       Upload File

I would like to apologize for the inconvenience. But we currently don't have a Spreadsheet widget that allows you to upload file on one of its columns.

By the way, there is request from before about adding an Upload File to Configurable list widget but there no estimated time for its implementation.

Also, I'm not that sure how would you like your form to look like.

Will it have multiple entries on the spreadsheet? Or the number of entries is only limited per submission? Are the column headers static and will require the same information over and over?
