Hello, thanks in advance for your help. I've tried searching through the forums for an answer but cannot seem to find a solution. I have this website http://surphaser.com/ with a form. For some reason the data is not showing up on my email even though when I test the form it works just fine. Can you provide any insight?

Thanks for the clue. The firstname.lastname@example.org email should work reliably for many various reasons one is that spam filtering rarely consider this email address.
With regards to your custom sender email, could it be due to your SMTP credentials? May I suggest to delete the SMTP email and try it again. See if that helps. You can also check on Mandrill if there are emails that are undelivered or pending. You should be able to see it on the Mandrill dashboard.
