Can I annotate on a user submitted form?

The heart of the question is my desire to receive a submitted form, and while speaking with the user I can them make notes about the information already provided.  I am hoping there is a widget or some other feature that I have overlooked that would allow me to configure this functionality into a form in predefined area prior to have a user filling out the form, and then after it has been submitted I would have a place where I could type information in based on what additional information the user provided during a conversation.

You can edit the submission made by the user by updating it using the submission page or adding a Edit Submission link in your Notifier email.
