Hi! I've been with Jot Form for years and generally have no problems. For the last couple months, we get calls all the time saying that they submitted the form, but when we search our system, we never got anything. It's strange because we do get some submissions, but it seems like it's becoming a consistent problem where for some reason we don't get random submissions.
Can you tell me if there is a known issue for this, or if there is something I can do on my end to prevent this?

I have inspected your form and I noticed that inside of Email Notification recipient email address is set to c*****@la*******.com.
I have checked that email address in our system and I saw that all submissions were sent successfully.
You can also check the email logs yourself:
Also your Email address is not in bounce list.
Please check spam, trash and bulk folders and if you find them there please mark them as not spam in order to not go into the folder anymore.
I would also suggest to whitelist JotMails IP Addresses: http://www.jotform.com/help/145-whitelisting-jotmails-ip-addresses
