My Account was suspended and I am not sure why. None of the forms will work

I see that your account was suspended due to the form titled "Update information" which is collecting passwords. It is not allowed to collect any sensitive information including passwords or create login forms through JotForm. Such forms are considered as phishing forms and associated accounts are suspended immediately. Please refer to our terms of usage for more information on phishing forms. Your account can only be activated only if you agree to our terms of usage and to delete the guilty fields from your JotForm. Please confirm if you want to us to delete the guilty fields from the JotForm and so that we can activate your account.
