Missing input field when Form is converted to PDF?

    Hi JotForm,

    I have tried to convert my form (link below), but realise that the pdf version omits some of the input fields. 

    Why are JotForm not able to convert correctly?

    (The two first essential input fields are omitted in the pdf version)

    Hi,

    Please check our guide regarding fillable PDF form: http://www.jotform.com/help/312-Fillable-PDF-Everything-you-need-to-know-about-JotForm-Fillable-PDF-Creator.

    As mentioned by the FAQs, these fields are widgets and are currently not supported by the fillable PDF creator:

    I would advise that you use the plain form fields instead. In this case, you can use the Date Time field.

    For the IMO number, you can use the plain text box and use the "Input Mask" to format the input of your user.
