Form Not Found after submittal

I have a form called K12 Bus ESP (1) that after an employee hits submit they get a response on screen that says the form is not found.  Can you help me resolve this please?

Hi,

This is because you have setup your form http://www.jotformpro.com/form/51944105317956, to redirect the user on this link http://form.jotform.us/form/50973290196159 when they hit the submit button, but that Form link not anymore active, you might have deleted that form.

To find it in your settings, you can follow this quick guide:

You'll see here that you have inputted that Form URL which is not anymore active.

If you want to redirect your user to another form, please get the correct link and add it again in your settings.
