Conditions not entirely being carried out

I just last night changed the layout and some of the content of my form. These changes were made because of the desired conditional rules. Some are working as intended. Some are not. Very basic conditions, like, show if "this" and require if "that". Could I please get some assistance with this problem? Thanks in advance

There are several conditions within the scenario desired. Some of which are
working just fine. For example: if the field "Destination type" is equal to
"CB Base Barracks" then insert this address here and require address line
2. Those conditional rules are working just fine. However, The following
rule are not: If the field "Destination type" is equal to "Hotel / Motel"
then show and require "Hotel / Motel Name". If the field "Destination type"
is equal to "Apartment / Condo" then show and require "Name of Establishment". If the field "Payment Method" is equal to "Credit / Debit" then show and require {clickto50}. I have more but I stopped writing them when I realized they were not being applied anymore

Please show this fields in your form for the condition to take effect.
