Form not showing on Chrome on Mac OS X

Hello - I have created a simple form that shows up and works properly on Safari, however when I load the page on a Chrome browser, my form does not show up. The form is built into Squarespace. It's on a trial account so you can access the page at https://carerawards.squarespace.com/ and choosing visitor login.

Never mind - I found an earlier thread from a couple of years ago that suggested to change the code so thu URL starts with https://secure instead of http://form - this has worked for me.
