    Hello,

I am not sure if I have understood your question correctly.

Do you mean to say that the form you have embedded in your web page using script embed code is not working anymore?

I did check few of your last edited forms and did not find any issue with that. Please share the webpage URL where you have embedded your form and we will take a look.

We will wait for your response.

    Thank you!

Sorry...I hit submit on my post before I was able to add a description.
Here's an example of a page where the embedded form is not showing:
    http://www.prescott.edu/explore/plan-a-visit/preview-day-registration-form2.php
I found that when I disabled our mmenu script (http://mmenu.frebsite.nl/),
the form would embed as expected. I didn't see any errors in the console
when the form wouldn't embed, however, so I don't know for sure if there is
    a conflict or not.
    Any suggestions? Much appreciated!

Have you tried using the iFrame embed code instead? Here's a guide on that: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.

When you used the iFrame, it should prevent any code conflict between your website and your form.

Do let us know if it works.

Since we have several dozen instances of javascript embedded forms, I was
hoping to fix the issue, rather than update all instances with iframe, but
I suppose that's my only option, then. Thank you for your time.

    Hello,

The only reliable and easiest way to resolve the code conflict issue is to embed your form using its iFrame embed code. You do not have to replace all your form's embed code. Test your embed forms and if you see any issue, use the iFrame embed code.

    Thank you!
