We followed the advice given above. Indeed, headers/text appear on the form. However, when we log out and then back in, we find that this feature does not "hold." We have to reset the show header/text feature. How do we PERMANENTLY show this change for a given form? Thanks.

It should remember the settings you chose on that form so that even if you logged out and log back in, the "Show headers and text" option should still be enabled. Please make sure that "Show headers and texts" option are checked and then log out from JotForm. Please clear your browser cache. After that, close and reopen the browser. Please log back in to JotForm and check if the "Show headers and texts" option is still checked.
In addition to that, I will clear your form cache so that your forms will be refreshed from our servers. Hope this works. Thank you.

That is correct, you need to click the options icon first and then enable the Show headers and texts and then click the options icon again in order to save the settings.
