Please try disabling the responsive option at the preference tab:
And add the Mobile Responsive widget a the bottom of your form:

Do you need to add the widget as well? If so, it is to be inserted at the bottom of the form above the submit button?
Why do you need to use both?

But I will reply here also so that the original poster might also get an idea on how the responsive options works.
So basically we have two options that you can use:
Option 1: Enabling the responsive options in BOTH the Form Designer Tool and in the "Preferences". This is mentioned in this guide: http://www.jotform.com/help/311-How-to-make-Forms-Mobile-Responsive
Option 2: Using the Mobile Responsive widget. You can add the widget anywhere, but to make it more organize, you can add it at the bottom.
You can test which one works for you but we recommend that you only use only one of the said options.
Can you look at this form? I've tried both options, and neither works at all. Pretty much one of your canned forms, but doesn't work at all on my iphone 4S
Hello Mike, I tested you form on an iphone4 emulator and it was displayed correctly in a responsive manner.
Could you kindly test it on a chrome browser on you phone and let us know of the outcome, also a screenshot of how it appears on your end will be quite helpful.
I see that the issue is related to the jotform plugin. I have a direct link on the home page, which went to a page with the form embedded and wasn't responsive. I've replaced that with a direct link to jotform.com/xxxxx which is mobile responsive.
But clicking the jotform button gives me this error, on Chrome as well as the Safari on my iPhone. Direct link looks fine as you show above.
