Contact form on Website unable to complete submissions

I created a Contact Form using a template, but when when submitted it still saying"Please wait..." and not submitting the form.  Previewed form work when submitted from your website.

The form is currently on a local server.

If the form will submit when not embedded but will not submit when embedded, try replacing your embed code with the iFrame code:

Something running in your page may be interfering with the functionality of your form and causing it to be unable to complete submission.
