Conditions aren't being respected in the embed version of my form

So, I have a few conditions that I made in this form

1.- Show QUINCEANERA INFORMATION if GET FREE TICKETS is selected

2.- show MISS COVER GIRL REGISTRATION if REGISTER FOR MISS COVER GIRL is selected

Unfortunately, these conditions aren't being shown in the attached URL, is there something that you can do to help me with this?

It appears that the issue is caused by conflicts between jQuery library with related scripts and our Prototype javascript library based form.

If possible please use an iFrame embedding method.

Getting the Form iFrame Code

It will load the form in a separate frame, so other elements will not affect the form code.
