I checked your account and form, both appear to be active and usable. Your other account however, was suspended for collecting game account information. We do not allow for the collection of account usernames for any purpose.
I removed the IVMU Name field from your contact form and restored your other account. It should now be active and usable. Keep in mind that adding a field such as that back to your form will likely result in your account being suspended once again.
