After I filled out the form (as a test) a copy hit my email folder which is correct, but it does not show my answer, just the question. How can I fix?

You must submit the form in actual to be able to received answers in the email.
Using on the Test Email button in the Notification Email will only send you test message with only the field names on it, and NOT answers.
