I have tested mentioned form and I was able to submit it.
Also I noticed that mentioned form is not located under your account NYYC. And account NYYCSO does not exist. However when I checked your username I noticed that there are lot of accounts coming form same IP address.
Did you provide correct link of the form? Which browser are you using? I also suggest to clear your browser cache.
