Cannot load 3rd page of form in an embedded application

Our website is built with Adobe Muse. We have embedded our application into one of our pages. We go to the internet to fill in the form, we get past the first 2 pages, and then get a blank 3rd page displayed in our browser. The form shows up fine in jotform.com. Our website is www.empoweredmc.org and the application is listed under the apply here button on the first page.

I checked your page and I was able to reproduce the issue with the third page:

I cloned the form to test it and was able to replicate the issue in the clone as well. 

From my testing it looks like "Scroll To Top" widget is the culprit.  You only need one in the form and there are multiple in the form currently.  When I removed all the widgets and added a single one at the bottom of the form, the form worked once again.
