I have cloned your form and I was able to reproduce the issue. However, I tried to add the widget from the Form Builder and the widget seems to be working fine.

The newly added SmartyStreet widget is working OK.

Can you please try removing the widget in your form and try adding it back using the Form Builder?
As my colleague had previously described, he was able to make it work by just adding a new widget to the form.
I checked the test form http://form.jotformpro.com/form/51993891278978? and was able to make the widget work on my browser as well.
Let us know if issue persist.
