Smartystreets widget is malfunctioning

I developed this widget. Now it is malfunctioning. I am attaching two files in this regard. Thanks.  

I have cloned your form and I was able to reproduce the issue. However, I tried to add the widget from the Form Builder and the widget seems to be working fine.

Can you please try removing the widget in your form and try adding it back using the Form Builder?

As my colleague had previously described, he was able to make it work by just adding a new widget to the form.

I checked the test form http://form.jotformpro.com/form/51993891278978? and was able to make the widget work on my browser as well.
