    JotForms has worked really well for me! I have never had ad an issue for 2/3 forms I have up on my website. Recently, two clients have submitted the Placenta Prep agreement and I am getting no notification of it and it's not showing up on JotForms either. I made sure they hit submit button... Please help! I've enjoyed JotForms so far, but need it to be very reliable.

    When I checked the embedded form http://www.jotform.us/form/50587789340164  in your website, I can see right away that there is an issue with script conflict in the form. Script conflict can cause issue and inconsistency on how the form will behave upon submission.

    What I suggest you try first is to re-embed the form to your website using its iframe embed code instead. Currently, you are using the default script embed.

    Make sure to clean/clear first the form's script code embedded before adding the iframe embed code to avoid any conflict.
