Create new row in excel output based on answer in one field

We would like for our form to produce an extra row in excel based on a particular input. For example:

If they answer "Issue#2", then we would like an extra row to be created in excel with the same "Address Field". Is this possible? They enter the address. Then, "Issue#1" with a yes or no answer. If there is more than 1 issue, we would like for JotForm to produce an extra row in excel but still carry the same address down.

We are trying to eliminate the user having to fill out 3, or 4 forms for the same address.

    Thanks,

    B

Well, let me ask one thing before we proceed:

Is there a way we could put a "Duplicate form" on the bottom of the form where they could press it, and it would automatically duplicate what they have on the current form, and open a new window with the same form? Duplicate it, but they could still

change an answer and then submit if they wanted to?

This question seems to be a different one and shall be addressed on a separate thread shortly to avoid confusion.
