My form appears to be disabled?

    In the enable/disable field, I enabled my form. I cannot locate the field that is causing my form to be inactive. How can I troubleshoot this now and in the future



    Form appears enabled...

    StatusEnable or Disable Form.

    I believe this is because of the word "username", we have our anti-Phishing system the regularly scans thousands of forms, and that field is a violation of our "Terms of Use", this mimics a phishing page that ask for sensitive information like log in credentials like usernames. 

    I've re-activated your account, but I would advise that you delete those fields and avoid creating similar ones to avoid being autosuspended again.
