My form is not showing any fields to fill out, only text fields and submit button

    When I took a look at your jotform, I did find it strange that only text and submit button were there, but looking at the conditions, I was able to see the issue.

    There are many "This field has been deleted" warnings, which by themselves can cause a lot of issues, while the actions that are there are all stating that you want the fields hidden.

    Basically this resulted in all of your fields being hidden.
