It's possible that there's a code conflict between your website and the form's widget.

Have you tried using the iFrame embed code instead? Here's a guide on that: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. That should prevent any code conflict and should load all the fields and widgets correctly.

Do let us know if that works.

hi thanks for the reply I tried the iframe option but it doesn't look very good as my form pages very so much in height that at times parts of the form are getting cut off when conditional logic shows extra questions. Im currently making a new page template in wordpress and removing as much of the code as possible that not needed on the form page in  a hope that i can find the conflicting scripts.

    thanks

If that's the case, you'll probably need to make separate pages for each of your form to avoid conflicts, if the iFrame is not an option.

If your on Wordpress, you might want to check this guide also: http://www.jotform.com/help/324-A-Guide-To-Forms-For-WordPress-JotForm-Users, see if the methods there will help.

Do let us know if that helps.
