I have attempted more than 10 times to upload the JPEG image of our registration form per your instructions. Any ideas of why it is not loading? Also, if I delete the previous images, it seems that they will disappear from last year's form. Is that correct?

Can you please try again? I just tried it both methods, adding the image from web URL of the image, and uploading it, and it worked:
You can try deleting and adding back the Image field, let us know what happens.
