How do I add a new field to existing jotform

How do I add email address and phone number to existing JotForm?

You have to access your My Forms page (https://www.jotform.com/myforms/) then click the form you want to add a field to it, click "Edit Form" option:

In the form builder, just simply drag and drop the Email Address and Phone field from the Quick Tools section:
