I've attached a form where the filled out fields doesn't match what is received on the email.
Date of Birth and the country or state of issue appears to be blank when it's been filled in the form.
Could you please investigate and help us resolve this?
May I know if the form is embedded in a website? If it is, could you share us the link to it? You might need to use the iFrame embed code if that's the case.
