How can I remove the "Sample Logo" on my form? I am not sure how I put it there, but I don't want that to appear. Thanks!

    On which form is the logo on, you can provide a link and we will help accordingly.

    You can also inject the following CSS in your form to remove the logo

    .form-all:before {
    background-image: none;
    }
