Aligning Form Inputs

I am tryint to add a product to my form named "Families & Addiction", however, instead of being added on a line of its own, it is going next to the "DBT" product. And I can only see it when I preview my form. I can not see the product when I am in the product list trying to edit it.

I checked your embedded form and did not see anything next to the DBT product other than that products options:

I was also able to add a new product below the DBT product and have it be on its own line:

The product that is next to DBT with the Jun 13-14, 2016 date is not supposed to be there. That is supposed to be a completely new product that is called Families & Addiction and should be on its own line.
