Why is my address question not showing up on the forms my customers are filling out?

    I cloned it and I see there's an address field with previous conditional logic on it.

    If you check your saved conditions.

    You'll see that you have fields deleted, this is causing the address not showing properly because the conditional logic is not met.

    You'll either need to delete the conditional logics if you do not need them or update them to work as expected.
