It appears as though your account was auto suspended by our anti phishing tool. Particularly this is because you used the terms "Username" and "Password" in two of your forms. These forms have been deleted and access to your account and other forms have been restored.
The purpose of the two forms that were removed appears to be used as custom thank you pages. JotForm has a built in feature in the Thank You Page wizard for this kind of behavior if you are interested in that feature.
Here is a demo form with a custom thank you message.
In order to add a Thank you Message to your form you can open your form in the form builder and click on the Thank You button. Then select "Thank You Message" and click next.
You can alter the message using the provided markdown editing tools. When you are finished click the button labeled "Finish" and when your form is submitted the user will see your thank you message.
