My Forms no longer display the Submist button when loaded?

Hi there,

There is no longer any submit button appearing at the end of any of my forms. There is also a footer with an image on the forms which is no longer displaying. Everything appears to be correct in the form editor, but when you try and load the form and fill it out, there is no submit button.
