Can we include social security number field on the form using its source code on Joomla website

    we are a mental health practice and require a social security number on our intake form.  If we do so, and use the secure form to embed the source code into a Joomla article, will this be acceptable within your terms of use?

    I recommend you create first the form (after creating an account) using this link http://www.jotform.com/signup . You can signup using your email account.

    Add the form into your Joomla website, and then we can evaluate further how the form with such requirement will be used.

    Hello Jonathan,

    Thank you for your prompt reply.  I have created an JotForm account.  After practicing with creating a form from scratch, I am now experimenting with modifying one of your existing templates; the Intake Form for Mental Health Providers

    I am slowly converting our current website (www.nicapc.com) to Joomla. 

    As you indicated, I will contact JotForm once I add the form to the website and go live.
